Child care personnel failed to immediately contact emergency services and the child's custodial parents or legal guardians in the event of a serious illness, accident, injury or emergency to their child. CCF Handbook, Section 6.3, B & C and 2.5.3, E Health Requirements [SR] Comments: An abnormality to the child's head was first noticed by staff at approximately 6:10 am. Staff observed an area of the child's head swell up throughout the morning. According to parents they were not notified until they received a phone call at 12:10 pm asking them to pick the child up. Staff's inability to identify a serious symptom of head trauma led to a delay in notifying the parents. On August 11, 2025, CPT medical staff subsequently diagnosed a healing skull fracture. To ensure that all staff will be able to identify the signs of head trauma and react in a timely and appropriate manner, provider will develop a facility policy on recognizing and preventing Shaken Baby Syndrome/Abusive Head Trauma (SBS/AHT) and conduct a staff training familiarizing staff with the contents of the policy. If the facility already has an SBS/AHT policy, refresher training will be conducted, and each staff member will be provided with a copy of the policy. Provider will email a copy of the facility SBS/AHT policy, as well as a copy of the agenda and sign-in sheet for the staff training to the licensing office on or before 1/5/2026.
