During the facilitys licensure year, fire drills utilizing the approved alarm system were not conducted monthly at various dates and times when children were in care [March 2026]. CCF Handbook, Section 3.8.4, A (Section 2.1 - Health and Safety, Page 3) Physical Environment [SR] Comments: The facility did not conduct a fire drill in March of 2026. Facility will conduct 2 drills in the month of April to be in compliance with drills. Provider will send updated log to licensing via email by 4/30/26. The play equipment was not maintained in a safe or sanitary condition for the children to use in that [on the three-year-old and up playground there is a piece of concrete at the bottom of the stairs. The pole is anchored down into it. The piece of concrete was accessible and unsafe to children.]. CCF Handbook, Section 3.12, B and F Sanitation and Equipment [SR] Comments: On 02/24/2026 a child was playing on the three-year-old and up playground when he fell and hit his head on a piece of concrete sticking out from the bottom of the playground equipment. The child went to the ER and received stiches on his forehead/over his right eyebrow. Per DCF CCF Handbook section 3.12: B. All playground equipment must be securely anchored, unless portable or stationary by design, in good repair, maintained in safe and sanitary condition, and placed to ensure safe use by the children. Maintenance must include inspections conducted every month of all supports above and below the ground and of all connectors and moving parts. Documentation of maintenance inspections must be maintained for 12 months. The concrete is not able to be removed due to the equipment being anchored into it.
