30U — Personnel Files

Ensure staff files are complete. Two staff need an orientation, two documented attempts to obtain service letters, and references. One staff person needs an orientation, OCCL's Health & Safety Training, an application/resume, and references.

40U — Child Files

Ensure child files are complete. One child needs a health appraisal. Three children need an updated health appraisal with immunizations. One child needs a blood lead level test result.

49U — Indoor Space

Ensure all sinks supply both hot and cold water. One sink in the girl's toilet room only had cold water. This was corrected at the visit when the hot water was turned on. Ensure sinks are in good repair. One sink in the girl's toilet room was clogged and not draining properly. This was corrected at the visit when the drain was repaired.

40U — Child Files

Ensure child files are complete. Two children need updated health appraisals and immunizations. Two children need blood lead level test results. Three children need updated health appraisals.

49U — Indoor Space

Ensure sinks supply both hot and cold water. The sink in the bathroom in room 104 only supplied cold water. This was corrected at the visit when the hot water was turned on.
