Staff #1 and #5 need complete file, including: CPR/First Aid, Administration of Medication, signed Professional Development Plan, Training hours (9 hours for PT, 18 hours for FT), Annual Abuse and Neglect, Orientation, OCCL Health and Safety Training, Application/Resume, Job Description, References (4 if no previous work experience, 2 if there are service letters signed), Declaration/Drug and Alcohol Statement, Adult Abuse Registry check, Health Appraisal with TB test, Release of Employment and Service Letters. Staff #2 and #3 need signed Professional Development Plan, training hours (9 for PT, 18 for PT), and Annual Review of Abuse and Neglect

38T — Parents Right to Know

Parents Right to Know Log must have updated parent signatures. Send updated Log to OCCL.

40T — Child Files

Child files must be complete. Child #3 needs: Emergency Card Info (including date enrolled and dates of attendance-- IE: M-F 3:30PM-5:30PM), Parents Right to Know, Screen Time Permission, Transportation permission and Medication Permission (these are on the Child Info Card) Health Appraisal: Child #5, #7, #11 Immunization Record: Child #5, #7, #11 Blood Lead Test Results: Child #2, #5, #6, #7, #8, #11

8T — License Renewal

Please send notarized renewal application and insurance information 60 days prior to license expiring.
