Ensure Personnel files are complete. Staff #5 needs: Job description, references, health appraisal with TB test, Release of employment and service letters Staff #6 needs DEEDS certificate, OCCL Health and Safety training, and Health appraisal with TB test. Staff #7 needs DEEDS certificate, OCCL Health and Safety training, signed Declaration, and signed Drug and Alchohol Statement Service letters when they are returned for Staff #1 New references for Staff #1, #2, #3, #4, #6, #7, #8, #9 (The concern is that all the references for the staff listed above are staff members already working at the center, and all of the letters say similar things about each applicant.)

72U — Feeding of Infants

Ensure infants are held to be fed- no propping bottles.

49U — Indoor Space

Put crown molding up/finish wall in the 3/4's classroom downstairs. Send picture to OCCL. Replace sink faucets in the 3's classroom upstairs and the paper towel holder.

45U — Hazardous Materials

Ensure radon is tested and sent to OCCL every 5 years.

40U — Child Files

Ensure child files are complete. Health Appraisal and Immunization records- Child #7 and Child #11
