Ensure staff files are complete 18 hours of annual training including 2 in health and safety. Training hours are needed for Staff #1 (18 hours), Staff #2 (18 hours), Staff #3 (18 hours), Staff #4 (18 hours), S Staff #5 (18 hours), and Staff #6 (9 hours) PD Plan are needed for Staff #1, #2, #3, and #6. Review of safe sleep and abuse and neglect is needed for staff #1, #2, #3, #6, and #9. 7-hour OCCL Health and Safety training is needed for Staff #4, #6, #7, and #11. Health Appraisal is needed for Staff #5 and #11. Orientation is needed for Staff #7 and #11 References are needed for Staff #4 (2 references), Staff #7 (4 references), Staff #10 (4 references), Staff #11(4 references) Send documentation to OCCL by February 6, 2026.

35U — First Aid and CPR Training

Ensure First Aid and CPR is completed within two months of hire and renewed every two years. Staff #4 and #7 need to complete First Aid and CPR. Send documentation to OCCL by February 6, 2026.

40U — Child Files

Ensure child files are complete Updated physical is needed for Child #3, #5, and #11. Immunization records are needed for child #3. Blood lead screening results are needed for Child #3 and #8. Send documentation to OCCL by February 6, 2026.

53U — Outdoor Area

Ensure gates are self-closing and positive self-latching closure. Add self-closing latch to all gates on fenced playgrounds. Send picture to OCCL by February 6, 2026.

Ensure child files are complete. Child #2 and #8 needs health appraisal Child #11 needs immunization records Child #5 and #11 need Blood lead screening Send documentation to OCCL by February 10, 2025.

42U — Documenting Children's Progress

Ensure children's progress is being documented. Child #3, #5, #10, #11, #14, and #15 need development plans. Send documentation to OCCL by February 10. 2025.

45U — Hazardous Materials

Ensure radon testing is completed every five years. Send documentation to OCCL by February 10, 2025.

63U — Administration of Medication

Ensure medication administration record is completed for all medications. Child #14 needs a MAR completed for the tube of diaper ointment. use the specific name of the ointment used.
