45U — Hazardous Materials

Ensure cabinets are locked or that hazards are not accessible to children. The infant classroom has an unlocked lower cabinet, and several choking hazards are accessible. The hazards include plastic-containing craft items, plastic-wrapped paper towels, clothespins, and ribbon.

40U — Child Files

Ensure child files are complete. Two children needed a date of enrolment. One child needs hours and days of care.

36U — Staff Attendance

Ensure the exact departure time is recorded. One staff member was signed into the Pre-K classroom at 8:40 a.m. but was not present. Attendance records for the toddler classroom show that the same staff member was signed in at 9:05 a.m.

30U — Personnel Files

Ensure staff files are complete. Four staff members need at least two hours in health and safety training. Two staff members need CPR/First Aid. Two staff members need a release of employment history form completed. Two staff members need documented attempts for service letters. Two staff members need two reference letters. One staff member needs a job description.

27U — Ratios, Group Size, and Supervision

Enasre qualified staff are supervising the infant classroom. There was an intern in the infant classroom. This was corrected when an early childhood teacher was placed in the infant classroom.

42U — Documenting Children's Progress

Ensure that child developmental plans are completed and updated at least three times a year. 11 children need developmental plans.

8U — License Renewal

Ensure an annual renewal application is submitted 60 days before the license's expiration.

72U — Feeding of Infants

Infants require a monthly feeding schedule and must be updated monthly. One child s monthly feeding form requires a monthly update; the last update was on May 1, 2025. One child needs a monthly feeding form.

63U — Administration of Medication

Ensure any medication on site has a Medication Administration Record (MAR) and is labeled with the child s name. One child requires a MAR for antifungal cream and Aquaphor. Thirteen children require an MAR for diaper cream. Four children require an MAR for lotion. Three items do not have a child s name or MAR.

61U — Diapering and Soiled Clothing

Ensure the diaper pad is nonabsorbent. The one-year-old classroom diaper changing mat has holes in it.

59U — Hand Washing

Ensure that children s hands are washed after meals. The one-year-old and two-year-old classrooms did not wash their hands after lunch.

30U — Personnel Files

Ensure staff files are completed. All staff members need to have professional development plans completed. One staff member needs an eligibility letter on file. One staff member needs Health and Safety for Child Care Professionals training, an adult abuse registry check, a health appraisal on OCCL s form, a release of employment history, and service letter attempts with follow-ups as necessary.
