Ensure staff files are complete. Staff #1 needs- Signed PD plan, the 7-hour OCCL health and safety training, two references, Health appraisal, and Deeds certificate. Staff #2 needs- An updated medication certification. (expires every 5 years) Send documentation to OCCL by January 9, 2026.

51T — Toilet Facilities

Ensure sink is in proper working order. The water did not turn on when LS and the Administrator tried. Sink working Send video of working sink to OCCL by January 9, 2026. .

25U — Substitutes, Volunteers, and Contracted Special Services Persons

Ensure an on-site files is created for each volunteer who volunteers at least 40 hours per year before volunteer begind to volunteer. volunteer without Deeds certificate is considered an aid and must be supervised at all times. -personal data sheet - Start adn end date -Health appraisal w/ TB results -Orientation - Review of abuse & neglect -signed declaration and drug and alcohol -verification of adult abuse registry check Send file to OCCL by January 10, 2024

41U — Child Health Appraisal

40 Child Files Ensure child files are complete. Child # 1, #6, and #10 need Blood Lead screening results Please send documentation to OCCL by January 10, 2025.
