45T — Hazardous Materials

Ensure radon tests are completed every 5 years. The facility's radon results expired 8/16/2025.

40T — Child Files

Ensure child files are complete. One child needs blood lead screening results. One child needs a health appraisal, immunization record, and blood lead screening results.

30T — Personnel Files

Ensure staff files are complete. Six staff members need CPR and first aid certification. Three staff members need administration of medication training and/or certification. Two staff members need annual review of abuse and neglect. One staff member needs an eligibility letter. Five staff members need professional development plans. Two staff members needs to complete OCCL's Health & Safety Training for Child Care Professionals training. One staff members needs a job description and a service letter or two documented attempts. One staff member needs two references and an adult health appraisal with TB results.

28T — General Qualifications

Ensure staff are fingerprinted and eligibility determination is on file before the start of employment. A staff member has not been fingerprinted for the facility. According to the administrator, the staff member is a kitchen staff without child care duties.

14T — Notification to OCCL

Ensure the Office of Child Care Licensing (OCCL) is called within one business day, and the caller must speak with someone, in the event a child requires outside medical or dental treatment other than any first aid provided at the center from an incident that occurs while the child is in care. On July 22, 2025, Administrator #1 sent LS #1 an email with an accident or injruy report for a child who required outside medical treatment for a pom pom that was lodged in the child's nose. The incident occured on June 25, 2025.

63T — Administration of Medication

Ensure outdated medication is returned to the parent. This was corrected when the meds were discarded of because the child no longer attends the program.

27T — Ratios, Group Size, and Supervision

Ensure to maintain staff to child ratio. Specialist observed a high school student alone in the hall escorting a child to the restroom. This was corrected when the specialist had a conversation with Adm Wilson regarding supervision.
