Ensure the annual renewal application is received by OCCL 60 days prior to the license expiration date. Received at visit.

44P — First Aid Kit

Ensure there is a complete First Aid Kit available. The kit was missing a thermometer.

30P — Fire Safety

Ensure fire drills are conducted every month. May was not conducted. Please provide OCCL with the June evacuation log.

22P — Children's Files

Ensure Child Files are complete. The following is needed: Current Health Appraisal--Child #1, 2, 3, 5 Current Record of Immunizations--Child #1, 2, 3 Blood Lead Screener Results--Child # 3, 5

8P — License Renewal

Ensure the annual renewal application is received by OCCL 60 days prior to the license expiration date. Due date for the application was 5/31/2025--corrected at visit when received by provider.

56P — Training

Ensure that annual training hours are completed 30 days prior to the license expiration date.

22P — Children's Files

Ensure Child Files are complete. Child #2 and #3 need current Health Appraisal/TB Screener, Immunization Records and Blood Lead Screener Results, as well as a Parent Permissions form. Child #4 needs Blood Lead Screening results.

21P — Parents Right to Know Act

Ensure the Parent's Right to Know log is filled out for all families touring the program and/or enrolling. Submit a log with all of the new children who began the program this past licensing year.
