The following deficiencies were observed at the time of the Annual Compliance Inspection conducted on 2/25/2025 and are subject to changes pending programmatic review. A paper copy of the Notice of Inspection Rights was provided at the time of the inspect
Compliance (Annual) The following deficiencies were observed at the time of the Annual Compliance Inspection conducted on 2/25/2025 and are subject to changes pending programmatic review. A paper copy of the Notice of Inspection Rights was provided at the time of the inspection. A Plan of Corrections will not be accepted at this time. The Emergency Disaster Contact form was completed at the time of the inspection. The DES Group Size Contact form was completed at the time of the inspection. The Empower Self-Survey was emailed to the provider following the inspection. Please complete it within 10 days of receipt. The fingerprint clearance cards for 1 of 2 staff and 1 resident were verified to be valid through the DPS website at the time of the inspection. During the exit interview, the following items were discussed but not limited to: Requirements for Emergency, Information, and Immunization Record cards and updating Criminal History Affidavit with a new fingerprint clearance card. Result: Complete Source: AZ Care Check (ADHS Bureau of Child Care Licensing).
