Compliance (Annual) The following deficiency was observed at the time of the Compliance Inspection conducted on 09/11/2024 and is subject to changes pending programmatic review. The Plan of Corrections will not be accepted at this time. The Empower Self-Evaluation survey was emailed to the licensee at the time of the inspection. The fingerprint clearance cards for 1 of 2 staff members were verified to be valid through the DPS website at the time of the inspection. The following was discussed but not limited to: DCS Central Registry check: Submit the application and print the results using the Arizona Centralized Background Checks website (cbc.az.gov). Fingerprint Clearance card: Use the Department of Public safety website for status of applications and verification of cards (azdps.gov). The Compliance Officer is Tara Farrell. Result: Complete Source: AZ Care Check (ADHS Bureau of Child Care Licensing).
