Compliance (Annual) The following deficiencies were observed at the time of the Annual Compliance Inspection conducted on 12/03/2025 and are subject to changes pending programmatic review. Please submit the Plan of Corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. The Compliance Officer provided a paper copy of the Notice of Inspection Rights at the time of the inspection. The fingerprint clearance cards for 2 of 2 staff members and 3 of 3 residents were verified to be valid through the DPS website at the time of the inspection. During the exit interview, the following items were discussed but are not limited to: *Criminal History Affidavit will be complete *Exposed raw wood behind the toilet will be repaired *Documentation of completed staff training will be available for review *Fence gate (side yard with lawnmower) will be closed when enrolled children are present in the outdoor activity area Result: Complete Source: AZ Care Check (ADHS Bureau of Child Care Licensing).
