   Complaint The purpose of the inspection was to conduct Complaint #00161607 investigation on 3/31/26. A full inspection was not conducted at this time. A focus inspection was conducted The Compliance Officer provided a paper copy of the Notice of Inspection Rights and the Small Business Bill of Rights to the Facility Director at the time of the inspection. Ratios observed were: 1 year olds: 1:3 2 year olds: 1:5 3/4 year olds: 2:7 There were 2 staff members interviewed during this investigation. There was a staff file reviewed during this investigation. There was a child’s file reviewed during this investigation. The Compliance Officer contacted the complainant by email on 3/10/26. Documentation observed was: communication documents from the parent, incident documentation and rosters. Upon completion of the complaint investigation, it was determined from observation, interview and documentation that (2) of (2) allegations were unable to be substantiated due to a lack of sufficient evidence. The following deficiencies were observed and are subject to changes pending programmatic review.
