   Complaint The purpose of the inspection was to conduct complaint #000167506 investigation on 4/28/26. A full inspection was not conducted at this time. The Written Documentation is required to be submitted through the Licensing Portal within 10 days of the receipt of the Statement of Deficiencies. Ratios observed were: Infants: 2:7 & 2:5 One year olds: 2:6 & 2:7 Two-year-olds: 2:3 & 3:11 Three & fours: 3:30 Four year olds: 1:14 There were 7 staff interviewed during this investigation. There were 3 staff files reviewed during this investigation. The fingerprint clearance cards for 3 of the 3 were verified to be valid through the DPS website. Others interviewed: The complainant. Documentation observed were rosters, written documentation regarding the incident, observation reports, accident/injury reports, children's emergency cards and children's attendance records. Upon completion of the complaint investigation, it was determined from observation, interview and documentation that 1 allegation was substantiated. 4 allegations lacked sufficient evidence and were unable to be substantiated.
