   Compliance (Annual) The following deficiencies were observed at the time of the annual inspection conducted on 09.15.25, and are subject to changes pending programmatic review. The Compliance Officer provided a paper copy of the Notice of Inspections Rights and the Small Bill of Rights to the facility director at the time of the inspection. A link to the Empower Self-Evaluation was sent to the facility director. The Emergency Disaster Plan update form was completed at the time of the inspection. Five fingerprint clearance cards reviewed were verified as valid through the DPS website. Please submit a current violation-free fire permit. Please submit the Plan of Corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. The following was discussed, but not limited to: *Ensure the splintered wood from the wooden arch is not accessible to children. *Ensure the wooden arch is secured to the ground and not wobbly. *Ensure toilet caulking is maintained in a clean and smooth/sanitizable condition. *Ensure toilet paper is mounted. *Ensure chipped glass is not accessible to children. *Maintain floor covering free from hazards. *Ensure copies of the back of staff members' fingerprint clearance cards are maintained in the staff files. *Obtain parental permission to sign students in and out. *Ensure the statement of services contains all required items. *Ensure the current license is posted.
