A focused inspection was conducted at this time. A paper copy of the Notice of Inspection Rights was given to the facility at the beginning of the inspection. Ratios observed were: Ones/Twos: 1:4 Threes to Fives: 1:9 Documentation observed: Emergency Cards and parent handbook. There were 2 staff members interviewed during the investigation. Upon completion of the complaint investigation for case #165910, it was determined from observation, interview, and documentation that the one allegation was substantiated. Please complete the Plan of Corrections via the LMS Portal within 10 days of receipt of this Statement of Deficiencies. Result: Complete Source: AZ Care Check (ADHS Bureau of Child Care Licensing).
