Compliance (Annual) The following deficiencies were observed at the Compliance Inspection conducted on 11/5/2025, and are subject to changes pending programmatic review. Please submit the Plan of Corrections via the LMS portal within 10 days of the receipt of the Statement of Deficiencies. A copy of the Notice of Inspection Rights was provided at the time of the inspection. 5 of 5 fingerprint clearance cards were verified to be valid through the DPS website during the time of the inspection. The following was discussed, but not limited to: 1) New rules, effective 8/3/25; 2) All current staff are required to submit clearance applications through the DCS portal, that was effective 7/2024; and 3) The refund policy posted is required to be conspicuously on the posting board. Please submit a copy of the gas inspection that is scheduled on 11/6/25, when obtained. Result: Complete Source: AZ Care Check (ADHS Bureau of Child Care Licensing).
