Ratios observed were: Ones and Twos - 2:12 Threes - 1:8 Threes and Fours - 1:13 Fours - 1:15 There were 6 staff member(s) interviewed during this investigation. There were 3 children interviewed during this investigation There was 1 staff’s file reviewed during this investigation. There was 1 child’s file reviewed during this investigation. Documentation observed was: Roster of the date of the incident. Upon completion of the complaint investigation, it was determined from observation, interview, and documentation that the 1 allegation was unable to be substantiated. There were zero deficiencies observed and are subject to changes pending programmatic review. 1 fingerprint clearance card was verified to be valid through the DPS website during the time of the inspection.
