   Compliance (Initial) The following deficiencies were observed at the Initial Inspection conducted on 9/25/2025, and are subject to changes pending programmatic review. Please submit the Plan of Corrections via the LMS portal within 10 days of the receipt of the Statement of Deficiencies. A copy of the Notice of Inspection Rights was provided at the time of the inspection. 17 of 17 fingerprint clearance cards were verified to be valid through the DPS website during the time of the inspection. The following items were discussed, but not limited to: 1) New rules, effective 8/3/2025; 2) Updating the room numbering plaques on the classrooms to match the room numbers used on the architect's plans; 3) Updating capacities in each room since the licensee is no longer requesting to self-limit each room's capacity; 4) Staff files items that need to be updated due to the change in ownership: TB attestation, DPS affidavit, DCS background check, Date of employment and Orientation training; and 5) Statement of services now must include the suspension and expulsion policies.
