   Complaint The following deficiencies were observed at the time of Complaint #00133056 and #00133061 Investigations conducted on 6/11/2025 and are subject to changes pending programmatic review. A full inspection was not conducted at this time. The Compliance Officer provided the facility with a paper copy of the Notice of Inspection Rights at the start of the inspection. Please submit the Plan of Correction in the Portal within 10 days of receiving the Statement of Deficiencies. Ratios observed were: Infants: 1:4 1s: 2:13 2s: 1:5, 2:12, 1:5 3s: 1:10, 1:10 4s: 1:11 There were 7 staff interviewed during this investigation. Documentation observed: children's attendance records, Emergency Information and Immunization Record cards, classroom rosters, staff schedules, and electronic messages with photos. The Compliance Officer contacted the complainant. Upon completion of Complaint #00133056 Investigation, it was determined from observation, interview, and documentation, that 2 out of 4 allegations were substantiated, and 2 out of 4 allegations were unable to be substantiated due to a lack of sufficient evidence. Upon completion of Complaint #00133061 Investigation, it was determined from observation, interview, and documentation, that 2 out of 2 allegations were substantiated.
