   Complaint The purpose of the investigation was to conduct Complaint 00171137 investigation on May 27, 2026. A focused inspection was conducted The Compliance Officer provided a paper copy of the Notice of Inspection Rights and the Small Business Bill of Rights to the Facility Director at the time of the inspection. Ratios observed were: Infants 1:4 Ones 1:1 Ones and Twos 1:5 Twos and Threes 1:5 Threes and Fours 1:9 Fours and Fives 2:10 There were 4 staff interviewed during this investigation. There were 3 staff files reviewed during this investigation. There were 2 children’s files reviewed during this investigation. Others interviewed: The Compliance Officer on Duty contacted the complainant on May 26, 2026 by phone. Upon completion of the complaint investigation, it was determined from observation, interview, review of video footage and documentation that 7 of 7 allegations were unable to be substantiated due to a lack of sufficient evidence.
