   Complaint The purpose of the inspection was to conduct Complaint #167654 and #169881 investigations on 5/15/2026. A focus inspection was conducted. The Compliance Officer provided a paper copy of the Notice of Inspection Rights and the Small Business Bill of Rights to the Facility Director at the time of the inspection. Ratios observed were: Infants: 2:6, 2:11 1s: 1:6 2s: 1:8, 1:8 3s-5s: 2:26, 1:12 There were 4 staff members interviewed during this investigation. There were 2 staff files reviewed during this investigation. Others interviewed: The Compliance Officer contacted the complainant (#167654) on 5/07/2026 by email. The Compliance Officer was unable to contact the complainant (#169881) on 5/15/2026 via email or phone. Documentation observed was: -Rosters -Incident and Accident reports -Written Observation notes Upon completion of the complaint (#167654) investigation, it was determined from observation, interview, and documentation that 2 of 2 allegations were unable to be substantiated due to a lack of sufficient evidence. Upon completion of the complaint (#169881) investigation, it was determined from observation, interview, and documentation that 1 of 1 allegation was unable to be substantiated due to a lack of sufficient evidence. The following deficiencies were observed and are subject to changes pending programmatic review.
