Compliance (Annual) The following deficiencies were observed at the Compliance Inspection conducted on 12/9/2025, and are subject to changes pending programmatic review. The Plan of Corrections will not be accepted at this time. A copy of the Notice of Inspection Rights was provided at the time of the inspection. The Emergency Disaster Contact form was emailed to the provider to complete. 7 of 7 fingerprint clearance cards were verified to be valid through the DPS website during the time of the inspection. The following items were discussed, but not limited to: 1. Ensure the facility has a violation-free fire inspection and send the Department documentation of the repairs once items are fixed. 2. General cleanliness of restrooms and plumbing fixtures. Result: Complete Source: AZ Care Check (ADHS Bureau of Child Care Licensing).
