   Complaint The purpose of the inspection was to conduct a complaint investigation for Case #00128096, Case #00128112, and Case #00128113 on April 24, 2025. A full inspection was not conducted at this time. Ratio observed was: 1 staff member present with 6 children, ages infant - school age. There were 7 staff members interviewed during this investigation. There were 6 children interviewed during this investigation. There were 2 staff files reviewed during this investigation. Documentation observed was: Facility's breakfast, lunch and snack menus and grocery receipts. Upon completion of the complaint investigation for Case #00128096, it was determined from observation, staff and child interviews, and documentation that 3 of 5 allegations were substantiated. Upon completion of the complaint investigation for Case #00128112, it was determined from observation, staff and child interviews, and documentation that none of the 3 allegations were able to be substantiated. Upon completion of the complaint investigation for Case #00128113, it was determined from observation, staff and child interviews, and documentation that 1 of 3 allegations was substantiated. The following deficiencies were observed and are subject to changes pending programmatic review. Two of two fingerprint clearance cards were verified to be valid through the DPS website during the time of the inspection.
