Complaint The following deficiencies were observed at the time of case # 00148878 investigation conducted on 10/28/25 and are subject to change pending programmatic review. A full inspection was not conducted at this time. Ratios observed were: infants: 1:2 1's: 1:6 2's: 1:8 3+: 1:8 There were 2 staff interviewed during this investigation The owner called in on 10/24/25 and notified the bureau of the incident. A Police report was created and will be provided once completed. Owner has copy of the Dog's immunization and rabies reports, will forward documentation. Documents reviewed included: Rabies and Immunization documents. Upon completion of the complaint investigation for case # 00148878, it was determined from interview that the allegation was substantiated.
