Complaint The following deficiencies were observed at the time of the complaint investigation conducted on 3/19/2026 for case #00162459 and are subject to changes pending programmatic review. Ratios observed were: Infants: 2:8 1-year-old children: 1:5 2-year-old children: 1:7 Preschool: 1:11 4 staff were interviewed at the time of the inspection. Others interviewed: Complainant and Director Documentation reviewed: Incident report, injury log, staff files, and tummy time log. Upon completion of the complaint investigation #00162459, it was determined from interviews and documentation that 2 of 2 allegations were substantiated. Compliance (Initial) The following deficiencies were observed at the time of the Initial Compliance Inspection conducted on 10/22/2025 and are subject to changes pending programmatic review. Please submit the Plan of Corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. The Notice Of Inspection Rights was provided to the licensee at the time of the inspection. The fingerprint clearance cards for 6 of 6 staff members were verified to be valid through the DPS website at the time of the inspection.
