    The purpose of the inspection was to conduct complaints #155704 & #155706 investigation on 1/22/26. A monitoring inspection was also conducted at this time. The Written Documentation is required to be submitted through the Licensing Portal within 10 days

    Complaint;Monitoring The purpose of the inspection was to conduct complaints #155704 & #155706 investigation on 1/22/26. A monitoring inspection was also conducted at this time. The Written Documentation is required to be submitted through the Licensing Portal within 10 days of the receipt of the Statement of Deficiencies. There were 5 staff members interviewed during this investigation. There were 2 staff files reviewed during this investigation. The fingerprint clearance cards for 2 of the 2 were verified to be valid through the DPS website. Others interviewed: The complainants. Documentation observed were rosters, discipline policy, the parent handbook, incident reports, including bite reports. The following was discussed but not limited to: Following up with the child's parent regarding if medical attention was sought for the 1/6/2026 injury to a child's head from a fall from a table while the child was at the childcare. Per R9-514.G., if medical attention was sought, the required information must be documented. Upon completion of the complaint investigation, it was determined from observation, interview and documentation that 3 of the 3 allegations lacked sufficient evidence to be substantiated. A paper copy of the Notice of Inspection Rights was provided to the Licensee during the inspection. The following was discussed, but not limited to: * Registration of Empower and completion of Emergency Disaster Preparedness forms following initial licensure; * Completing all staff file requirements following initial licensure; * Ensuring all children's emergency cards are updated as needed; * Ensuring playground equipment is maintained in a clean condition; * Ensuring children and staff follow sun safety guidelines; * Ensuring seats in the facility's motor vehicle are maintained free from hazards; * Ensure procedures are followed when diapering children with special health care needs; * Completing all required practice drills; * Posting of all allergies and/or modified diets in each activity area and the kitchen; * Ensuring food served to children is of a size appropriate for the age and development of the child; * Ensuring that restrooms are maintained in a clean condition and free from hazards.
