   Complaint;Initial Monitoring The following deficiencies were observed during the Initial Monitoring Inspection and Complaint #158004 Investigation conducted on 03/10/2026, and are subject to changes pending programmatic review. A full inspection was not conducted at this time. The Compliance Officer provided a copy of the Notice of Inspection Rights to the Facility Director at the time of the inspection. Ratios observed were: Infants: 1:3 1s/2s: 1:4 3s-School Age: 2:12 There were 3 staff interviewed during this investigation. The following documents were reviewed during this investigation: Attendance Records, Rosters, written staff statements, and incident/injury reports Upon completion of the complaint investigation #158004, it was determined from observation, interview, and documentation, that 1 of 1 allegation was unable to be substantiated due to a lack of sufficient evidence. The following deficiencies were observed and are subject to changes pending programmatic review. Please submit the Plan of Corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. During the exit interview, the following items were discussed, but are not limited to: *Hot water (86°F-110°F) will be accessible for handwashing at the diaper sink. The hot water valve was in the off position at the time of the inspection. *The torn laminate and exposed pressed wood on the art sink in the Young Achievers will be repaired or inaccessible if children are present. *Supervision means: for an enrolled child, knowledge of and accountability for the actions and whereabouts of the enrolled child, including the ability to see or hear the enrolled child at all times, to interact with the enrolled child, and to provide guidance to the enrolled child.
