   Compliance (Initial) The following deficiencies were observed at the time of the Initial Compliance Inspection conducted on 9/22/2025 and are subject to changes pending programmatic review. Please submit the Plan of Corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. The Compliance Officer provided a paper copy of the Notice of Inspection Rights to the provider at the time of inspection. The fingerprint clearance cards for 13 of 13 staff members were verified to be valid, and 1 of 13 was determined to be not valid through the DPS website at the time of the inspection. There was one vehicle approved for transportation (CK19337). The following was discussed but not limited to: 1). Ensuring there is enough shade on the playground for all children. 2). Ensuring that plastic/rubber on the outside of play structures is maintained in a non-hazardous condition. 3). Ensuring that all personal items have the child's first and last name on them.
