Complaint The purpose of the inspection was to conduct a complaint investigation. The complaint was a self-report. A full inspection was not conducted at this time. The following deficiencies were observed at the time of complaint #00165940 investigation conducted on 6/11/2026 and are subject to changes pending programmatic review. A Written Plan of Corrections will not be acceptedat this time. The Notice Of Inspection Rights was provided to the licensee at the time of the inspection. Ratios observed were: Infants: 1:3 1-year-old children: 1:4 1 and 2-year-old children: 1:5 2-year-old children: 1:8 3-year-old children: 2:9 3 and 4-year-old children: 2:18 There were 5 staff interviewed during this investigation. There was 1 staff file reviewed during this investigation. Documentation observed were staff statements, rosters, incident reports and a police report. Upon completion of the complaint investigation #00165940 , it was determined from observation, interview and documentation, that the allegation was substantiated. Compliance (Initial) The following deficiencies were observed at the time of the Initial Inspection conducted on 10/7/2025, and are subject to changes pending programmatic review. Please submit the Plan of Corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. The fingerprint clearance cards for 13 of 13 staff members were verified to be valid through the DPS website at the time of the inspection.
