    Compliance (Initial) The following deficiency was found at the time of the Initial Licensure Inspection conducted on 04/09/2026, subject to changes pending programmatic review. The Compliance Officer provided the facility with a paper copy of the Notice of Inspection Rights at the start of the inspection. Six of six fingerprint clearance cards were verified to be valid through the DPS website during the time of the inspection. The facility was not licensed at the time of the inspection. Items discussed, but not limited to, were as follows: -Plungers and toilet brushes must remain inaccessible -Fingerprinting and background check timeframes -Signing in and out for children and staff -Children’s Emergency, Information, and Immunization Record card completion -Staff: files, start date, and training (10-Day and Annual) -Discipline and guidance procedures -Children with Special Needs: Individualized Plan -Mandated Reporting of suspected abuse and neglect -Medication (form, storage, labels, rescue meds, etc.) -Supervision (nap, outside, and inside) -Fire safety: scheduled annual inspections with Fire Marshal or local Fire Department. -Contact Compliance Officer when any changes are being made to the facility, Director position, or facility closure on a normally scheduled day - Change date of enrollment in Emergency Information and Immunization Record Cards to match the license certificate starting date. - Ensure the second parent is marked N/A on Emergency Information and Immunization Record Cards. - Ensure immunization records are attached to the Emergency Information and Immunization Record Cards. - Change date of employment on staff files to match the license certificate starting date. - Print DPS verification page to verify validity of Fingerprint Clearance Cards prior to start employment.
