Complaint The following deficiencies were observed at the time of Complaints #00151512 and #00151505 investigations conducted on 12/5/25 and are subject to changes pending programmatic review. A telephone call was made to the Complainant on 12/5/25 and an email was sent on the same day. The Written Document of Corrections is due within 10 days A complete inspection of the facility was not conducted. The following classroom ratios were observed: Infants: 2:7 Ones: 2:13 Twos: 2:15 Threes/Fours/Fives: 2:19 Threes/Fours/Fives: 2:16 Threes/Fours/Fives: 3:18 Five staff members were interviewed during this investigation. The following documentation was reviewed: Classroom rosters Upon completion of the complaint investigation, it was determined from observation, staff interview and documentation that 4 of 4 allegations lacked sufficient evidence to be substantiated.
