   Complaint The following deficiencies were observed at the time of Complaint #00171448 investigation conducted on 5/29/26 and are subject to changes pending programmatic review. A full inspection was not conducted at this time. ***The complaint was self-reported by the facility on 5/28/2026. Please submit the Plan of Corrections on the Licensing portal within 10 days of receiving this Statement of Deficiencies. The following ratios were observed: Infants: 1:4 1's: 2:6; 1:6 2's: 2:8 3's: 1:10; 2:10 4's: 1:15 5's; 1:15 Two staff members and one student were interviewed during this investigation. The following documentation was reviewed at the time of the inspection: Written Statements, Incident Reports, Staff File Upon completion of the Complaint investigation, it was determined from facility documentation and staff interview that 1 of 1 allegation was substantiated. Steps taken by Facility: *Facility Director ensured that the incident was reported to AZDHS within the required timeframes and a Police Report was submitted to Goodyear Police. *Staff #1 was terminated on 5/27/26 *Facility Director has scheduled a staff-required training on 6/8/26 titled: Positive Guidance and Discipline for Infants Through School-Age Children Result: Complete Source: AZ Care Check (ADHS Bureau of Child Care Licensing).
