Complaint The following deficiencies were observed at the time of the complaint investigation conducted on 10/18/24 for case #00091378 and are subject to changes pending programmatic review. A full inspection was not conducted at this time. Please submit the Plan of Corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. Ratios observed were: School-age children - 1:3, 2:5 3-5-year-old children - none were present at the time of the investigation. There were five staff interviewed during this investigation. Documentation reviewed: Facility report documentation, rosters. The Compliance Officer spoke with the Compliance Coordinator via telephone and email on 10/10/24 regarding their report submitted to the Department relating to an incident on March 5, 2024. Upon completion of the complaint investigation #00091378, it was determined from observation, interview and documentation, that 1 of 1 allegation was substantiated.
