Compliance (Annual) The following deficiencies were observed at the time of the Compliance inspection conducted on 8/8/2025 and are subject to changes pending programmatic review. The Compliance Officer provided the facility with a paper copy of the Notice of Inspection Rights at the start of the inspection. 5 of 5 fingerprint clearance cards were verified to be valid through the DPS website during the time of the inspection. Please submit the Plan of Corrections via the LMS portal within 10 days of the receipt of the Statement of Deficiencies. The following was discussed but not limited to: 1. Bird feces on the turf and climbing structure. 2. Staff files must be complete and should include previous child care experience. 3. The menu must reflect the specific items to be served to enrolled children. 4. Medication forms must have specific reasons to administer medications. 5. Defrost the freezers. 6. The weekly menu must be posted. 7. Cleanliness of the diaper sink in the 1s room. 8. The current license must be posted. Result: Complete Source: AZ Care Check (ADHS Bureau of Child Care Licensing).
