Compliance (Annual) The following deficiencies were found at the time of the Compliance Inspection conducted on 5/22/2025 and are subject to changes pending programmatic review. The Written Documentation is required to be submitted through the Licensing Portal within 10 days of the receipt of the Statement of Deficiencies. The following was discussed but not limited to: 1) The Date of employment, as it relates to timeframe requirements for staff obtaining a fingerprint clearance card, TB test, licensee verifying fingerprint clearance card’s validity etc; 2) Items stored in a bathroom are required to be stored inaccessible in either in a closed cabinet or in a covered, waterproof container stored on a shelf; and 3) Reminder of the license anniversary fee due date. There were 4 staff files reviewed. 4 of the 4 fingerprint clearance cards were verified to be valid through the DPS website.
