Compliance (Annual) **AMENDED** The following deficiencies were observed at the time of the Compliance Inspection conducted on 10/09/2024 and are subject to changes pending programmatic review. Submit the Plan of Corrections using the AZDHS Licensing Portal within ten days from the date the Statement of Deficiencies is received. The Empower Self-Evaluation survey was emailed to the licensee at the time of the inspection. The fingerprint clearance cards for 5 of 5 staff members were verified to be valid through the DPS website at the time of the inspection. The following was discussed but not limited to: DCS Central Registry check: Submit the application and print the results using the Arizona Centralized Background Checks website (cbc.az.gov). Director change: Update the Director using the AZDHS Licensing Portal. Criminal History Affidavits: Complete all Criminal History Affidavits to include the CDC or address. The Compliance Officer is Tara Farrell. Result: Complete Source: AZ Care Check (ADHS Bureau of Child Care Licensing).
