Compliance (Annual) The following deficiencies were observed at the time of the Compliance Inspection conducted on 9/23/2025 and are subject to changes pending programmatic review. A paper copy of the Notice of Inspection Rights was given to the Director at the time of the inspection. Please submit the Plan of Corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. The fingerprint clearance cards for 5 of 5 staff members were verified to be valid through the DPS website at the time of the inspection. During the exit interview, the following items were discussed, but are not limited to: *Continue to monitor the gaps created by erosion at the base of the playground fence to ensure they remain less than 4 inches. *Maintain 6 inches of wood chips in the fall zones of the playground equipment Result: Complete Source: AZ Care Check (ADHS Bureau of Child Care Licensing).
