Compliance (Annual) The following deficiencies were observed at the time of the Compliance Inspection conducted on 9/25/2025 and are subject to changes pending programmatic review. Please submit the Plan of Corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. The Emergency Disaster Contact form was completed at the time of the inspection. The Empower Self-Evaluation was emailed to the director. The Notice Of Inspection Rights was provided to the licensee at the time of the inspection. The fingerprint clearance card for 8 of 8 staff members was verified to be valid through the DPS website at the time of the inspection. During the exit interview, the following items were discussed, but are not limited to: *Ensure the required areas have 6 inches of resilient surfacing. *Ensure staff members sign in and out. *Ensure changing tables are located in the correct location. *Ensure the first aid kit contains the required items. *Ensure illness and injury log shows parent was contacted. *Ensure tissue is stored properly. *Ensure handwashing sink meets the required temperature.
