Complaint The purpose of the inspection was to conduct a complaint #00149481 investigation on 11-19-2025. The following deficiencies were observed at the time of the inspection. Please submit the Plan of Corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. A full inspection was not conducted at this time. A copy of the Notice of Inspection Rights was provided at the time of the inspection. The ratios observed were: Room 7 (4s)- 1:6 Room 8 (School-age)- 1:18 Cafeteria (School-age)- 1:17 2 staff members were interviewed during this investigation. Documentation observed was: Child write-ups. Upon completion of the complaint investigation, it was determined on observation, interview, and documentation that 2 of 2 allegations were unable to be substantiated due to a lack of sufficient evidence. The following was discussed, but not limited to: 1. Inaccessibility of lamps when not in use. 2. Exposed bulbs on lamps made inaccessible when in use.
