Complaint The following deficiency was observed at the time of the Complaint Investigation (Case 00144213 and 00145233) conducted on 10/16/2025, and are subject to changes pending programmatic review. A complete inspection of the facility was not conducted. Submit the Plan of Corrections using the AZDHS Licensing Portal within ten (10) days from the date the Statement of Deficiencies is received. The following staff-to-children ratios were observed: Ones/Twos: 2:10 Threes: 2:9 Threes/Fours: 1:9 Fours/Fives: 1:12 The Department contacted the complainant, who did not respond, on 09/08/2025 and 09/17/2025. 4 staff members and the Director were interviewed Documentation reviewed: Classroom Rosters for 08/11/2025-08/22/2025. Upon completion of the Complaint Investigation (Case 00144213 and 00145233), it was determined from staff/Director interviews, facility documentation and the Compliance Officer's observations two allegations were substantiated.
