   Compliance (Annual) The following deficiencies were observed at the annual compliance inspection conducted on 03-11-1016, and are subject to changes pending programmatic review. Please submit the Plan of corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. A copy of the Notice of Inspection Rights was provided at the time of the inspection. The Emergency Disaster Contact form was completed at the time of the inspection. BCCL staff emailed the Empower Self-Evaluation Assessment link to the Provider. 3 of 3 fingerprint clearance cards were verified to be valid through the DPS website during the time of the inspection. The following was discussed, but not limited to: 1. Staff file emergency contact documentation requirements. 2. Criminal History Affidavit completion. 3. 10-day new staff orientation training documentation requirements. 4. Annual staff training documentation requirements. 5. Staff file good faith effort reference documentation. 6. First-aid kit components. 7. Illness/Injury log documentation requirements. 8. Main posting area requirements. 9.
