    The following deficiencies were observed at the time of the Annual Compliance Inspection conducted on 4/10/26 and are subject to changes pending programmatic review. A full inspection was conducted. Please submit the Plan of Corrections via the LMS portal

    Compliance (Annual) The following deficiencies were observed at the time of the Annual Compliance Inspection conducted on 4/10/26 and are subject to changes pending programmatic review. A full inspection was conducted. Please submit the Plan of Corrections via the LMS portal within ten days of receipt of the Statement of Deficiencies. A copy of the Notice of Inspection Rights was provided at the time of the inspection. The Emergency Disaster Contact form was emailed to the Provider at the time of the inspection. The fingerprint clearance cards for 5 of 5 staff members were verified to be valid through the DPS website at the time of the inspection. During the exit interview, the following items were discussed but not limited to: *Lesson plans must be dated for the current week. *Infant feeding plans must be complete. *CBC results must be in each staff file. *Toxics, chemicals, and hand sanitizer must be inaccessible to enrolled children. *Outlets must be covered with a safety plug or insert. *Cords must not pose a tripping hazard. *The sticker on the diaper surface must be removed. *Faucets must be secured. *Peeling rubber on the playground must be secured. *Burr grass must be removed from the field if children use the field. *Refrigerators must be maintained at 41 degrees F or below. *Specific reasons for medications must be listed on the medication form. *Peeling paint in the toilet room.
