   Complaint The purpose of the inspection was to conduct Complaint #172513 investigation on 6/10/2026. A focus inspection was conducted. The Compliance Officer Supervisor provided a paper copy of the Notice of Inspection Rights and the Small Business Bill of Rights to the Facility Director at the time of the inspection. Ratios observed were: Infants-1s: 3:3 1s-3s: 1:8 3s-SA: 3:16 There were 7 staff members interviewed during this investigation. There were 7 staff file reviewed during this investigation. Others interviewed: Facility Director The Compliance Officer contacted the complainant on 06/05/2026 by phone, and the Compliance Officer Supervisor contacted the complainant on 06/08/2026 by email. Documentation observed was: *Fire Inspection Permit *Staff Files *Fire Drill Log *Diaper Changing Logs Upon completion of the complaint investigation, it was determined from observation, interview, and documentation that 2 of 5 allegations were substantiated, and 3 of 5 allegations were unable to be substantiated due to a lack of sufficient evidence. The following deficiencies were observed and are subject to changes pending programmatic review. A Plan of Correction is not accepted at this time. A Provider Meeting will be scheduled.
