Compliance (Annual) The following deficiencies were observed at the time of the Annual Compliance Inspection conducted on 1/5/2026 and are subject to changes pending programmatic review. A paper copy of the Notice of Inspection Rights was provided at the time of the inspection. Please submit the Plan of Corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. The Emergency Disaster Contact form was given to the director at the time of the inspection. The fingerprint clearance cards for 7 out of 7 staff members were verified to be valid through the DPS website at the time of the inspection. During the exit interview, the following items were discussed, but not limited to: *Ensure Criminal History Affidavit forms have front and back. *Reviewed new and updated DHS forms. *Ensure the lunch menu is posted. *Ensure cleaning tools(brooms) are inaccessible. *Update Compliance Officer (photos) when construction work is complete. During the exit interview, the following items were discussed but not limited to: Fire Inspection Report; Emergency, Information, and Immunization Record forms; Immunization records; Children's files maintenance; Updated Criminal History Affidavits; Fingerprint Clearance card documentation and employment dates; Tuberculosis testing documentation; 10-day new hire orientation; Training Documentation; Good-faith references; Staff file maintenance; Food Service permit requirements; Supervision during meals; Staff’s personal items; Maintain cleanliness of wooden toys and furniture; Thermometers for refrigerators and freezers; Tummy time procedures; Infant Feeding Forms; paint on outdoor fencing; Staff attendance records; medication procedures; Children’s sign in & sign out records; and facility name change procedures.
