   Compliance (Annual) The following deficiencies were observed at the time of the Compliance Inspection conducted on 9/22/2025 and are subject to changes pending programmatic review. Please submit the Plan of Corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. The Emergency Disaster Contact form was completed at the time of the inspection. The Empower Self-Evaluation was emailed to the director. The Notice Of Inspection Rights was provided to the licensee at the time of the inspection. The fingerprint clearance card for 4 of 4 staff members was verified to be valid through the DPS website at the time of the inspection. During the exit interview, the following items were discussed, but are not limited to: *Ensure required documents are signed by the director. *Ensure required documents are signed before the starting date of employment. *Ensure good faith efforts and the contact dates are documented. *Ensure the main posting board contains the required documents. *Ensure personal products are labelled. *Ensure the diaper changing table and the surrounding surfaces are clear of unrelated items. *Ensure the soiled clothed bin contains a liner. *Ensure facility is free of toxic plants. *Ensure all climbing equipment is located on resilient surfacing, *Ensure the designated person can locate documents and assist with administrative duties.
