    Initial Monitoring The following deficiency was found at the time of the Initial Monitoring Inspection conducted on 05/19/2026, and is subject to changes pending programmatic review. The Compliance Officer provided the Small Group Home with a paper copy of the Notice of Inspection Rights at the start of the inspection. Five of five fingerprint clearance cards were verified to be valid on the DPS website during the time of the inspection. The following was discussed but not limited to: -12 or more annual training hours -Maintaining Staff and Resident files -Notifying the Department of changes or closures -Staff must document start and end times each day -Post current menu at beginning of week Please complete the Plan of Correction via the LMS Portal within 10 days of receipt of this Statement of Deficiency.     Compliance (Initial) The following deficiency was found at the time of the Initial Licensure Inspection conducted 03/27/2026, subject to changes pending programmatic review. The Compliance Officer provided the child care group home with a paper copy of the Notice of Inspection Rights at the start of the inspection. Three of three fingerprint clearance cards were verified to be valid through the DPS website during the time of the inspection. The child care group home was not licensed at the time of the inspection. Items discussed, but not limited to, were as follows: -Maintain documentation of artificial turf resiliency -Plungers and toilet brushes must remain inaccessible -Immediate access to group home must be granted -DES and Food Program enrollment -Closure application for existing license/ownership -Keep gate between kitchen and dining room area closed when children onsite -Fingerprinting and background check timeframes -Signing in and out for children and staff -Children’s Emergency, Information, and Immunization Record card completion -Staff: files, start date, and training (10-Day and Annual) -Discipline and guidance procedures -Children with Special Needs: Individualized Plan -Mandated Reporting of suspected abuse and neglect -Medication (form, storage, labels, rescue meds, etc.) -Supervision (nap, outside, and inside) -Field trips: Anytime children leave licensed address -Fire safety: Extinguishers renewed yearly and fire/emergency drills monthly -Contact Compliance Officer when any changes are being made to the group home, or if closed on a normally scheduled day Please complete the Plan of Correction via the LMS Portal within 10 days of receipt of this Statement of Deficiency.
