   Complaint The purpose of the inspection was to conduct complaint #155726 and #155730 investigation on 6/2/2026. A full inspection was not conducted at this time. The Written Documentation is required to be submitted through the Licensing Portal within 10 days of the receipt of the Statement of Deficiencies. Ratios observed were: Infants: 1:1 One-year-olds: 1:4 Two-year-olds: 1:3 Three & Four-year-olds: 2:5 School-Age Children:1:2 There were 4 staff interviewed during this investigation. There were 2 staff files reviewed during this investigation. The fingerprint clearance cards for 2 of 2 were verified to be valid through the DPS website. Others interviewed: Licensee Documentation observed was: Rosters Upon completion of the complaint investigation, it was determined from observation, interview, and documentation that 1 of 1 allegation for complaint #155726 and 1of 1 allegation for complaint #155730 were substantiated. The following deficiencies were observed and are subject to changes pending programmatic review. During the exit interview, the following items were discussed, but are not limited to: Ensure Director and Designee are posted on the main posting board.
