Compliance (Annual) The following deficiency was observed at the time of the Annual Compliance Inspection conducted on April 24, 2026, and is subject to changes pending programmatic review. The Compliance Officer provided the Group Home with a paper copy of the Notice of Inspection Rights at the start of the inspection. 5 of 5 fingerprint clearance cards were verified to be valid through the DPS website during the time of the inspection. The DES group size was evaluated at the time of the inspection. Please complete the Plan of Corrections via the LMS Portal within 10 days of receipt of this Statement of Deficiencies. The following items were discussed, but not limited to: - Emergency Information and Immunization Record Cards. - Screen time posting. - Safe sleep practices. Result: Complete Source: AZ Care Check (ADHS Bureau of Child Care Licensing).
